Overview
The Transition to Young Adult Special Interest Group (SIG) within the UKKA supports healthcare professionals in the kidney community who are involved in guiding young people with kidney disease through the complex shift from paediatric to adult services. This transition can be particularly challenging due to the differing structures and approaches of these services, adding stress to what is already a difficult period of growth and change for young patients. The group’s work prioritises developing and sharing resources aimed at easing this transition, facilitating shared decision-making between patients, families or carers, and healthcare professionals, and addressing issues that arise during this critical stage. Current initiatives include the development of digital bundles and pathways, such as the "Moving on Up Together 16+ pathway," creating shared decision-making frameworks that engage multi-professional teams from both paediatric and adult care, and producing informational videos to introduce adult care teams.
